TITLE: Locum Worker – Home Care Worker

LEVEL:  H3

RESPONSIBLE TO: Home Care Manager

LOCATION: Unit HG01, 24 Highbury Grove, N5 2EA

PURPOSE

To provide a home support service to people with enduring mental health needs within their own home.

RESPONSIBILITIES

1.             Service to Clients

1.1           Establish good working relationships with clients and maintain a high standard of professional practice in all aspects of your work.

1.2           Provide home visit to clients as directed by the Manager.

1.3           In line with assessed need and agreed Care Plans, provide practical support to clients with enduring mental health needs in their own homes as required:-

1.4           Purchase shopping, collect prescriptions and benefits and arrange for bills to be paid if necessary and encourage development of budgeting skills where appropriate;

1.5           Provide and assist with preparation and cooking of meals;

1.6           Assist clients with cleaning and maintaining their accommodation;

1.7           Be prepared to clean and maintain a client’s accommodation in their absence e.g. admission to hospital;

1.8           Prompt and encourage clients to keep medical leisure educational or other (e.g. Day Centre, GP, DSS) appointments and escort the client as necessary;

1.9           Personal hygiene - assist with washing, bathing, shaving.

1.10         Assist with letter writing, filling in forms, arranging medical and other appointments;

               
2.             Teamwork

2.1           Work within all relevant legislation such as Health & Safety, notifying any problems to the Manager, or in their absence the Director Care and High Support Services, at the earliest opportunity. Be fully conversant with Umbrella’s Health & safety Policy.

2.2           Throughout the performance of your duties conduct yourself in a manner which demonstrates respect for your workplace being a client’s home and aim to realise Umbrella’s philosophy of maximum user involvement in all aspects of your work.

3.             Administrative and Financial Duties

3.1           Maintain accurate records relating to clients with due regard to confidentiality and to the clients’ right of access to their personal file.

3.2           Ensure that all petty cash expenditure is accurately recorded and reconciled.

4.             Liaison

4.1           Ensure professional relations and good channels of communication with all departments within the organisation and colleagues in other Umbrella projects.

4.2           Maintain appropriate efficient and professional links with other agencies and carers, at all times adhering to Umbrella’s confidentiality policy and aiming to promote the best interest of the clients.

4.3           Under the direction of the Manager, attend ward rounds and client case reviews.

5.             General

5.1           Uphold and implement Umbrella’s Managing Diversity and Equality Policy.

5.2           Develop and maintain professional relationships and good channels of communication with colleagues and external agencies.

5.3           Act at all times in the best interest of Umbrella, drawing any malpractice to the attention of the Manager or Director Care and High Support Services.

5.4           Work flexibly and be prepared to perform other duties commensurate with the role.
